Peripheral Vascular Intervention (PVI) Quality Improvement Initiative is a prospective, multi-center, multidisciplinary registry that represents a regional collaborative effort to assess and improve quality of care and outcomes of patients with peripheral vascular disease who undergo percutaneous peripheral arterial intervention.
This state-of-the-art website was one of my largest projects. It's a secured website for collecting protected patient information, used in multiple hospitals all over the state of Michigan.
It has several multistep CCK forms for collecting all that data, with a scheduling system to notify the healthcare professionals when to call the patient for follow-up information. Each follow-up form has its own multistep content type that links back to the main form to track changes in the patient's health history.
All the information is synchronized nightly to an Oracle backend, where the data is further normalized and statistical data is generated. The resulting data is then sent back to the website for reporting purposes, from where the users get detailed information on the quality of their practices.
